Home Health Nurse – LPN
- Job Description: As a home health nurse – LPN,
- Responsibilities will be to assess patients’ condition and initiate start of care face-to-face visits, perform evaluation tasks including vital signs and medication review, administer medication as prescribed by the patient’s physician dress (or re-dress) wounds, provide patient education, coordinate with the interdisciplinary team regarding care plan progress, and make recommendations for devices/tools/medications that might improve patient’s quality of life.
- Job Requirements: Must be 18 years or older and have successfully completed a FL-approved LPN program from an industry-accredited institution; Must have reliable transportation and proof of auto insurance. Must successfully complete a Level II background check prior to offer and initiation of training. Must complete a competency check and all orientation prior to initiating patient care. Current FL State LPN license is required.
